    In the central part of the reverse is a relief image of the Collegiate Church in Ivano-Frankivsk. Above it is a horizontal line dividing the field into two unequal parts. Above the line are the city's coat of arms and its name in Ukrainian: «IВАНО-ФРАНКIВСК». Below the line is an inscription in Ukrainian: «350 років», meaning «350 years». Along the left and right edges of the coin is a decorative pattern of crosses.
  • Obverse
    In the central part, within a smaller circle, is an image of the Ivano-Frankivsk Town Hall. Below it are the Kyiv Mint mark and the year of issue, 2012. To the left of the town hall is the coat of arms of Ivano-Frankivsk, to the right is the inscription: «ПЯТЬ ГРИВЕНЬ» - 5 Hryvnias. To the left, above, and to the right of the inner circle is a decorative pattern of crosses. In the upper part, along the edge of the coin, divided by the coat of arms of Ukraine, is an inscription in Ukrainian: «НАЦIОНАЛЬНИЙ БАНК УКРАЇНИ», meaning «National Bank of Ukraine».
